I'm not sure if there's a mod for Dark Souls that dresses characters up. Here's a few things I've found if you'd like to get started with modding and potentially make a fix.

you can use free tools like GIMP (similar to photoshop)(you'll have to get the DDS plugin) and Blender, a 3d Modeling software. I have very little experience with Blender.
Here's another way you could also extract the textures for editing. This is link to a thread about getting the textures by what the games loads, meaning you'd have to be somewhere that the immodest character would be in game. It's like a memory dump that helps you grab the textures.

I can't say that I have ever played Dark Souls. I've just never had any interest in it. Best of luck with it. Perhaps this might get you a start in modding.
I could test out the fallout 3 mod for ya. It's one I own. I'll try to check it out and post my findings here. Are you looking for a no blood mod in general or specifically this one? This one seems to be made to work with a few other mods or mod packs. If you're playing without those larger mod packs you could simply use this one:

Wow, I'm honored to meet you. I haven't played much of Fallout 3 yet. I need to make a mod for it before I play through it. I've been busy working on a no swear mod for Fallout 1 and 2. I've finished testing fallout 1 and have started edits on the second game.
Do you know much about how to use the Geck? I've had some troubles trying to understand it. Most games I've done mods for usually have some sort of text files that I can reference, but the geck seems so foreign to me. It feels like it would take forever to do edits for. I'm used to being able to get to lots of files quickly and easily to do edits and repack them and put them back into a game. Not every game works that way, but many of them do.
I haven't had a chance to test that mod yet. I had issues reinstalling the game so I've not got to it yet. I'll get to it and post the results.

Crysis 1 only had a CD check. Crysis Warhead had limited activations, though you can use the de-authorization tool put out by EA to reclaim an activation. You can find it here http://activate.ea.com/deauthorize/. If I'm not mistaken, the game had 5 activations before you had to call support, but they eventually moved this to 50(I think). If you've installed the game on your current system without reinstalling your machine you can actually reclaim an activation. I can't remember if you have to have the game installed before doing this or not. Either way try it out. It tells you how many activations you have left at the end of the de-authorization wizard.
One thing to remember when installing is to make sure to patch them up to the current version. My mod might not work correctly otherwise because some of the files don't exist without the patch. Crysis Warhead also had an annoying software piece that would check the release date for the game and the date you're installing it to ensure it wasn't a stolen copy. That software tries to reach out to a server that no longer exists. After patching the game, this software was removed. Without patching it will never let you play.
