Very rarely. Occasionally I'll do some odd cross between UML, flowcharts, and pseudocode when trying to architect a solution, but rarely.

Haven't used a framework when I worked with PHP, some of those weren't even available at the time. Kind of wish they were, they seem pretty sweet.
Closest thing I used was ADOdb, for interfacing with the MySQL db.
(but that's a library)
I'd probably want to check them out if I ever pick PHP back up. Any you recommend?
As for frameworks in general, generally I'd just work with the .NET framework (and there are usually in house libraries and the occasional 3rd party).
But did you know what I learned today? Apparently I've been misusing the term "framework" the entire time and that the .NET framework might not technically be a framework... my world is destroyed.
